I think it has already rolled down, judging from the way she is holding it! She’s not just covering nip, she’s uplifting it and pressing it in to hide it from the camera. You can tell there’s a lot more to that tattoo under that wrinkle of skin, too(the dark area appears to be the rest of the tattoo).

I think it’s Atlas - holding the world on his shoulders? But in a few years, it’s going to be knees and feet sticking out from under the boob. She can get another tat then, on the boob, saying “Help me!”
